You must
bring all current vaccine history to your appointment. If
you do not have a copy please call your vet and request a
copy, you can have it faxed to the number listed above. |

|
|
All dogs
must be current on Rabies, Distemper, and kennel cough
vaccinations, have a negative heartworm test, be on
heartworm preventative, have negative fecal examination, and
have a current does of a topical flea preventative applied. |
|
|
|
|
|
|
|
If your pet is not
current we are able to do this at the time of your visit. |
|
|
|
|
|
|
|
All cats
must be current on Rabies, Distemper and Leukemia
vaccinations, have a negative Leukemia / FIV test, a
negative fecal examination, and have current dose of a
topical flea preventative applied. |
|
|
|
|
|
|
|
We
recommend bringing your own food to feed your pet, changing
diets often leads to diarrhea. We do have regular food
available. We also carry Science Diet Prescription Foods;
there is an additional fee for this. |
|
|
|
|
|
|
|
Please do not bring any
bedding toys, or dishes. We use stainless steel and provide
bedding and toys for sanitary reasons. |
|
|
|
|
|
|
|
If your pets requires
daily medication there will be an additional fee. |
|
|
|
|
|
|
|
We
recommend bathing all dogs before they leave our facility, a
complimentary bath is included with 5-days or more of
boarding. If your pet requires a full grooming please
inform us, our groomer can provide you with a quote. |

|
|
|
|
|
Twenty
minutes of playtime is included in each day of boarding.
Additional time is available for $5.00 for each additional
15 minutes. |
|
|
|
|
|
|
|
Because a
boarding facility / veterinary clinic is much different then
home, there is a chance your pet may be exposed to
contagious illnesses, refuse food, soil itself, incur
diarrhea, weight loss, or a dull coat. Please also be
aware that Boarsh Veterinary Clinic does its best to prevent
any of these and make your pets stay here as comfortable as
possible. |

I also
understand that in the event I cannot be reached at the
emergency contact numbers listed, Boarsh Veterinary Clinic
will take all reasonable measures to ensure the health and
safety of my pet. |

For first
time patients boarding with Boarsh Veterinary Clinic there
is an office visit charge. |
|
|
|
|
|
|
|
An
examinations fee is charged for animals not current on
vaccinations. |
|
|
|
|
|
|
|
A
boarding fee is charged for the first and last day,
regardless of what time the animal is picked up. |
|
|
|
|
|
|
|
Please
call before picking up your pet during normal business
hours. |
|
|
|
|
|
|
|
Pickups
are not available on Sundays. |
